COVID-19 Vaccination During Pregnancy Shows No Increased Risk for Mother or Child
A thorough analysis of 25 studies involving over 450,000 pregnant women vaccinated against COVID-19 with Pfizer-BioNTech’s mRNA vaccine reveals reassuring results. No increased risk of miscarriage, congenital malformations, preterm birth, hypertensive disorders, small for gestational age, low birth weight, or neonatal death was observed. The findings remain consistent regardless of whether vaccination occurred in the first, second, or third trimester, or based on the number of doses received.
Pregnant women are particularly vulnerable to severe forms of COVID-19, with an increased risk of ICU hospitalization and obstetric complications. Yet, vaccine hesitancy persists due to unfounded concerns about fetal or infant safety. This systematic review, based on diverse observational data, confirms that the vaccine does not cause major adverse effects for the mother or child.
The included studies cover diverse populations and different healthcare systems, with varied methodologies: national registries, prospective or retrospective cohorts. Despite these differences, effect estimates remain close to neutrality, with no warning signals. For example, the rates of congenital malformations among vaccinated women are comparable to those in the general population, and no link has been established between vaccination and delivery before 37 weeks.
Some limitations remain, such as the lack of precise data on first-trimester exposures or extremely rare events. However, existing large national studies have not identified any specific issues related to early vaccination. Variations in outcome definitions or exposure windows have not called into question the overall conclusion: this vaccine has a strong safety profile during pregnancy.
This synthesis strengthens public health recommendations encouraging vaccination for pregnant women. It enables healthcare professionals to reassure patients about the absence of demonstrated risks to their pregnancy or baby, while emphasizing the importance of protection against an infection that does carry proven dangers. The current evidence, though observational, is robust enough to support this preventive approach.
